Abstract
For the treatment of long penile and peno-bulbar strictures, many techniques had been described such as, pedicled penile skin, scrotal skin or free grafts. Among these, distal fasciocutaneous penile flaps gets increasing attention due to its hairlessness, the possibility of correcting long strictures in a single step and that it contains pedicule, rich in vascular structures.
Between 1997 and 1999, two patients with extensive anterior urethral strictures were treated by the distal circular fasciocutaneous penile flap technique. One of the patients was 21 years old and the other one was 63 years old. In both patients, the strictures were at the peno-bulbar urethra and they were 12 and 14 cm long, respectively. During the 12 and 30 months of follow-up period one of the patients had developed urethral fistula but it was cleared off spontaneously 2 months later. The same patient had developed glandular hypostasis in the first six months. However, it had disappeared after the 6th months. Stricture or diverticulum was not developed. Urinary flow rate was determined as 20 and 25 ml/sec respectively.
Despite the limited number of cases, treatment of long anterior urethral strictures by the circular fasciocutaneous penile flap technique appears to be a good one, due to its acceptable complication rate and its ability to treat extensively long strictures in a single step.
