Abstract

This study aimed to assess the diagnostic significance and incidence of perineural invasion, mucinous fibroplasia, and glomerulations and to assess the association between Gleason score, clinical stage, and these morphological features on prostate needle biopsy.

Slides from 135-sextant needle biopsy specimen that were performed between 1993 and 1999 were reevaluated. The study included 83 sextant needle biopsy specimens diagnosed as prostatic adenocarcinoma. Statistical analysis was performed to test the correlation between the morphological features and Gleason score and clinical stage.

Thirteen (15.6%) of 83 needle biopsy specimens with cancer showed perineural invasion, 3 cases (3.6%) showed mucinous fibroplasia and 6 cases (7.2%) showed glomerulations features. Ten percent of the cases were Gleason score=4, 74% Gleason score=5-7, and 16% Gleason score=8-10. The distribution of the cases according to the clinical stage was cT1 (15.3%), cT2 (46.1%), cT3 (30.7%) and cT4 (7.6%). The mean age was 63.3 years (range 65 to 74). The mean follow-up was 17.5 months (range from 6 day to 38 month). There was no statistically correlation between Gleason score and clinical stage with perineural invasion, musinous fibroplasia and glomerulation features.

These finding may be the result of the majority of the cases being high grade and advanced stage. The glomerular
